The “hybrid” in hybrid DPC actually refers to the existence of two distinct payment options, instead of a “hybrid” payment method wherein patients pay both a membership fee and use their insurance to cover care. It’s the practice itself—with a panel made up of both DPC patients and fully-insured patients using their insurance.

Nancy Cooper, APRN-CNP, is the newest provider at Reliant Health & Wellness, PLLC located at 822 W. Randolph. She accepts all major insurances! She is dedicated and committed to partnering with her patients to optimize health and prevent or reverse chronic diseases utilizing both traditional medical and holistic approaches.

Cooper earned her Master of Science degree from University of Oklahoma College of Nursing. She is also currently pursuing post-graduate certification in Functional Medicine from the Institute of Functional Medicine through the Cleveland Clinic Medical Center. Nancy is dedicated to partnering with her patients to optimize health and prevent or reverse chronic diseases utilizing both traditional medical and holistic approaches. Telehealth, both video and phone visits, are readily available and encouraged at this time.

Reliant limits their practice to a few hundred patients per doctor. This means that your provider spends more time with you when you need him or her. You’ll receive unlimited office visits so you can forget scheduling appointments weeks in advance. Typically you’ll get a same or next-day appointment – and it’ll be free.

They offer high-quality care to kids, adults, and geriatric patients. This includes chronic disease management (diabetes, high blood pressure, etc), mental health, urgent care, sports medicine, and annual physicals.

Minor labs and procedures, like minor suturing, cryotherapy, and skin biopsy are all included in your membership fee. If there is an extra charge for a service, you will know exactly what it is before proceeding. There are special labs and procedures that may require an additional fee. Members receive a discounted price for these special labs and procedures.

Reliant dispenses medications in-house, so they can offer medications to you at greatly discounted price. For many medications, the price they offer is less than your co-pay at a pharmacy. If you take medications for chronic conditions, the savings on medications alone may be greater than your entire membership fee.
